Students and Parents,

We will pass out consent forms next week during hands only CPR training for those who would like to get the HPV, Meningitis ABCWY, and/or TDaP vaccines that were discussed in yesterday's presentation. Vaccine administration day will be April 15th. I will have a signup sheet during CPR training. Students on Medicaid, CHIP, uninsured, underinsured (have insurance but does not cover vaccines) are eligible.
