Please fill out all information with the link above. When you submit the information, someone will contact you in the summer with a time and date to bring documents to central office. Please make sure your email and phone are accurate. Your registration will not be complete until you bring your documents. Please ensure you have the following:
*Birth Certificate
*Social Security Card
*Immunization Record
*Parent/Guardian photo ID
*Proof of Residency with parent/guardian name (ONLY: Water, light, gas, deed or lease agreement)
*Proof of Income (PK Only– last two current paystubs)

Favor de completar toda la información en el enlace de arriba. Cuando haya enviado toda la información, será contactado por personal de JISD durante el verano para informarle el día y la hora para entregar sus documentos en las oficinas centrales. Por favor asegúrese que su correo electrónico y teléfono son los correctos y actuales. Su registro NO podrá ser completado hasta que traiga sus documentos el día y la hora indicados. Asegúrese de tener lo siguiente:
*Acta de nacimiento
*Tarjeta de seguro social
*Cartilla de vacunación
*Identificación con foto de Padres/Guardianes
*Comprobante de domicilio a nombre del padre/guardián (SOLO: agua, luz, gas, contrato de escrituras o arrendamiento)
*Comprobante de ingresos (solo para PK – los últimos dos y actuales)
